Alarm Code Form

STACC is monitored 24/7 with cameras and has a 24/7 monitoring system. In addition to motion sensors, there are several alarms on doors. The alarm system at STACC has a primary alarm panel located next to the lower level exterior door. An access card form must be completed in conjunction with this form. Do not share your alarm code with anyone. Directions for using the alarm system will be provided. Please complete the below form and allow 3 business days to be processed.
